Behavioral Health/ Physical Health Chest Pain and Chronic Disease Management Program Behavioral Health Case Manager I

Location: REMOTE

The Behavioral Health Case Manager I supports our members with serious substance abuse, substance abuse-related disorders, and/or mental health-related illness and is responsible for performing case management, telephonically, within the scope of licensure for these members.

Primary responsibilities include, but are not limited to:

  • Use appropriate screening criteria, knowledge, and clinical judgment to assess member needs.

  • Supporting Commercial Behaviorla Health Case Managent Nationwide

  • Conduct assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.

  • Monitor and evaluate the effectiveness of the care plan and modifies the plan as needed.

  • Support member access to appropriate quality and cost-effective care.

  • Coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.

Qualifications

Required Qualifications:

  • Requires a current, active, and unrestricted LCSW, LICSW, LPC, LMFT, LMHC, or clinical psychologist license (as allowed by applicable state law), from the state in which you reside.

  • Requires MS or MA in social work, counseling with a broad range of experience with complex psychiatric and substance abuse, or substance abuse disorder treatment; 3+ years of clinical experience, or any combination of education and experience, which would provide an equivalent background.

  • Telephonic and/or face-to-face behavioral health case management experience is required.

Preferred Qualifications:

  • Experience and or training wtih Chest Pain and Chronic Diseases strongly preferred.

Applicable to Colorado Applicants Only Annual Salary Range*: $58.320-$72,900 Actual compensation may vary from posting based on geographic location, work experience, education and/or skill level. * The hourly or salary range is the range Anthem in good faith believes is the range of possible compensation for this role at the time of this posting.
